Forms PPTC 463A, PPTC 463B and PPTC 463C specify the documents that must accompany the application, as well as the required information, depending on the applicant’s relationship with the child. The forms also specify who qualifies as a guarantor, depending on who is applying, and the information the guarantor must provide.
Forms PPTC 463A, PPTC 463B and PPTC 463C must always be accompanied by form PPTC 155 — CHILD GENERAL PASSPORT APPLICATION for Canadians under 16 years of age (in Canada or in the USA).
All sections of form PPTC 463A, PPTC 463B or PPTC 463C must be filled out.

If the applicant is:
complete only form PPTC 155.
However, if the parent who is applying is not the legal guardian or does not have parental authority of the child, the following additional documents must be submitted along with the completed application form and all other requirements:
a letter from the child’s legal guardian or the person who has parental authority authorizing the applicant to apply for the child’s passport and authorizing the release of the passport to him or her;
a copy of the court order or agreements relating to the involvement of Family Services with the child if such an order or agreements exist. In the event that a copy of the court order or agreements cannot be forwarded to Passport Canada for confidentiality reasons, the passport application must be accompanied by a summary of the order or agreements signed by the director of Family Services or the agency’s lawyer. The summary must indicate:
the child’s legal guardian or the person who has parental authority;
whether a mobility restriction clause exists, and if so, what it entails; and
the date and number of the court order, if possible.
in the case of a probationary adoption, a letter from the director of Family Services confirming that the child is undergoing an adoption process, and the name of the future adoptive parents;
in the case of a private probationary adoption, the custody or guardianship agreement signed by the child’s biological parents and the future adoptive parents for the duration of the adoption process, if such an agreement exists.
If the applicant is:
complete only the following sections of form PPTC 155:
Section 1—Child's Personal Information;
Section 2—Information on Applicant
the first two questions; and
the information on the Applicant (parent or legal guardian) and Other parent or legal guardian—to be completed by the child’s parent or legal guardian (other than the director of Family Services or the adoptive parent(s)), if they retain custody or parental authority, or if they share custody with the adoptive parents;
Section 4—Proof of Canadian Citizenship (except the subsection Declaration of Applicant); and
Section 6—Credit Card Information, if applicable.
NOTE: Depending on his or her relationship with the child, the applicant must submit, in addition to PPTC 155, one of the following forms: PPTC 463A, PPTC 463B or PPTC 463C.
Note: If your circumstances require you to submit one of the following forms: PPTC 463A, PPTC 463B or PPTC 463C all of the sections of the form in question must be completed by the appropriate person.
